Medication Management
enABLE’s Medication Management Program is offered to individuals who require or desire assistance with the management of their medications. Our Registered Nurses will assess and reconcile your medications, organizes and fill your medication sets, re-order, drop off and pick up your prescriptions and communicate thoroughly with your medical providers to ensure medicines are working and controlling symptoms appropriately. Through monitoring visits, your nurse will monitor health status changes and will contact your doctor immediately. The goal of this program to provide you and your family with the peace of mind you are taking the right medication, the right dosage, at right time according to your Physicians orders.
Overseen by a Nurse Care Manager, our Medication Management Program addresses your needs, ensuring
- Weekly, Bi-weekly or monthly medication filling
- Customized medication management service based on clients regimen and needs
- Training, counseling and educating client or caregiver on administering medications
- Communication and collaboration with your medical care professionals
- Pharmacy communication, drop off and pick up
- Medication adherence and compliance oversight
